REFERRING CLINICIANS

For referring clinicians

A short but information-dense referral: clinical question, prior procedures, available imaging and the decision that needs specialist input.

Important: This content is for education and information organisation; it does not provide an individual diagnosis or treatment plan. Clinical decisions depend on history, examination and appropriate testing.

Referral summary builder

An organisational tool for clinicians. It does not automatically send or server-store the entered data.

Complex or recurrent fistula

  • Defined clinical question.
  • Previous operation notes.
  • Pelvic MRI and prior imaging when available.
  • Openings and secondary extensions described on imaging.
  • Crohn’s disease or suspicion.
  • Baseline continence before further intervention.

Obstructed defecation or incontinence

A functional description is more useful than a medication list alone: blockage sensation, digitation, leakage type, obstetric/operative history and prior functional testing.

What we do not ask for

We do not encourage indiscriminate testing. Referral quality improves when the clinician states the decision or question that needs specialist input.
